What is a PPE issue register?

A PPE issue register is the signed record of what personal protective equipment has been given to each worker, in what size, on what date, and in what condition. It is a custody document rather than a stock report: the entry that matters is the one where a named worker signs to confirm they received a specific item, were shown how to use and check it, and understand when it has to be replaced. Alongside the issue it records replacements, the reason for each one, and the return of items when a worker leaves or changes role.

Providing protective equipment is only part of the duty. Work health and safety regulations require a business to provide PPE that is suitable, that fits the worker, that is maintained and replaced, and to make sure it is actually used, while workers have their own duty to wear it and to report damage. None of that is demonstrable from a purchase order. The issue register is what connects the item to the person, which is why it is the first document asked for after an incident where protective equipment either failed or was not being worn.

What to include in a PPE issue register

This PPE issue register covers 10 key areas:

  • Worker name, employee or contractor number, role, crew or department and start date
  • Item issued: type, description, make and model where it matters, and any standard it is certified to
  • Size and fit details, including the result and date of any fit test for respiratory protection
  • Date issued, quantity, and whether the item is new or a replacement

How to use this PPE issue register

  1. Work out what is required before you issue anything: Base the issue on the risk assessment for the work rather than on a standard starter pack. Issuing the wrong protection is worse than issuing none, because it creates confidence without protection, and it is the outcome an inspector notices first.
  2. Fit the item to the person: Check size and fit with the worker present rather than handing over a box. For respiratory protection, complete and record a fit test, and note anything, such as facial hair, that affects the seal and the choice of respirator.
  3. Give instruction at the point of issue: Show the worker how to fit the item, how to check it before each use, how to clean and store it, and when it must be replaced. Tick the instruction box only when this has actually happened, not as a matter of routine.
  4. Have the worker sign, and sign it yourself: The worker signs to confirm receipt and that they understand their duty to wear and look after the item. The issuer signs too, so the entry is attributable at both ends rather than resting on one signature.
  5. Record replacements with a reason: Every replacement gets its own line with the reason: wear, damage, loss, expiry or a change of task. The reasons are the useful part, because a pattern of loss or early wear usually points at storage, specification or the task itself.
  6. Review expiry dates and close out leavers: Check the register regularly against expiry and expected replacement dates so nothing stays in service past its life. When a worker leaves or changes role, record what was returned, its condition and what was disposed of.

How often should you complete this log / register?

Make an entry at every issue, replacement and return, without exception. A register with gaps is worse than no register at all, because it implies a system that was not followed.

Review the whole register at least quarterly against expiry and expected replacement dates, and more often where the work involves respiratory protection or fall arrest, since both have hard service lives and both fail quietly. Re-check fit whenever a worker changes role, where the task or hazard changes, and after any significant change in the worker that could affect the seal or fit of the equipment.

Under the model Work Health and Safety Regulations, a business that directs the carrying out of work must provide personal protective equipment to workers unless it has been provided by another business, and must make sure it is selected to minimise risk, suitable for the nature of the work and the hazard, a suitable size and fit, reasonably comfortable, maintained, repaired or replaced, and used or worn. Workers have a matching duty to use it as instructed, not to misuse it, and to report damage or defects. A signed issue register is the practical evidence that the provision, fit, instruction and replacement parts of that duty were actually carried out.

An issue register is a custody record: it shows which item went to which worker, when, and that they were instructed in its use. An inspection checklist is a condition record: it shows that an item still meets its acceptance criteria and is safe to keep using. You need both. The register answers who has it, and the inspection answers whether what they have is still fit for service. For items with a defined service life, such as harnesses and helmets, the two records are usually reviewed together.

A signature is not the legal test, but it is the most practical evidence there is. Without it you can show that equipment was purchased and issued in general terms, but not that a specific person received a specific item on a specific date and was told how to use it. That distinction matters when protective equipment is found not to have been worn. A digital acknowledgement with a timestamp works just as well as ink, provided it is attributable to the individual.

It depends on the item. Consumables such as respirator filters, and life-safety equipment such as harnesses, lanyards and helmets, generally carry a manufacturer service life or expiry that applies regardless of apparent condition, and they must come out of service when it is reached. Other items, such as gloves and general eye protection, are replaced on condition. Record whichever applies against each entry, and check the manufacturer instructions rather than assuming, because service lives vary between products.

Record anyone you actually issue equipment to, including contractors, labour hire workers and visitors given site protection. Where a contractor supplies their own equipment, note that on the register instead of leaving the line blank, because a blank looks like an omission during an audit. If you rely on another business to provide protection for its own workers, make that arrangement explicit in writing rather than assuming it.
